== Side effects == People taking opicapone very commonly (18%) experience dyskinesia. Other common side effects (in 1 to 10% of patients) include dizziness, strange dreams, hallucinations, constipation, dry mouth, orthostatic hypotension (low blood pressure), and muscle spasms. Apart from spasms, these side effects are also known from tolcapone and entacapone. As with entacapone, no relevant liver toxicity has been found in studies. This is in contrast to the first COMT inhibitor tolcapone, which could cause – in some cases lethal – liver insufficiency.
In collaboration with the Academic Medical Center in Amsterdam, Inreda Diabetic B.V. has developed a closed loop system with insulin and glucagon. The initiator, Robin Koops, started to develop the device in 2004 and ran the first tests on himself. In October 2016 Inreda Diabetic B.V. got the ISO 13485 license, a first requirement to produce its artificial pancreas. The product itself is called Inreda AP, and soon made some highly successful trials. In October 2020 the health insurance company Menzis and Inreda Diabetic then started a pilot with 100 patients insured by Menzis. These are all patients that face very serious trouble in regulating their blood glucose levels. They now use the Inreda AP instead of the traditional treatment. Another large scale trial with the Inreda AP was set up in July 2021, and should determine whether Dutch health insurance should cover the device for all their insured. === Transdermal === There are several methods for transdermal delivery of insulin. Pulsatile insulin uses microjets to pulse insulin into the person, mimicking the physiological secretions of insulin by the pancreas. Jet injection had different insulin delivery peaks and durations as compared to needle injection. Some diabetics may prefer jet injectors to hypodermic injection. Both electricity using iontophoresis and ultrasound have been found to make the skin temporarily porous. The insulin administration aspect remains experimental, but the blood glucose test aspect of "wrist appliances" is commercially available Researchers have produced a watch-like device that tests for blood glucose levels through the skin and administers corrective doses of insulin through pores in the skin. A similar device, but relying on skin-penetrating "microneedles", was in the animal testing stage in 2015. In the last couple of years, the use of chemical enhancers, electrical devices, and microneedle devices has shown tremendous promise for improving the penetration of insulin compared to passive transport via the skin. Transdermal insulin delivery shows a more patient-friendly and minimally invasive approach to daily diabetes care than the conventional hypodermic injection however, additional research is necessary to address issues such as long-term use, delivery efficiency, and reliability, as well as side effects involving inflammation and irritation.
=== Carbon metabolism === Clostridium scindens can anaerobically ferment several different carbon sources, including monosaccharides (fructose, galactose, glucose, mannose, ribose, and xylose), a disaccharide (lactose), and a couple of 2-sugar alcohols (dulcitol and sorbitol). Glucose metabolism takes the form of mixed acid fermentation, as the fermentation products include acetate, ethanol, and formate. In defined and minimal media, the preferred glucose fermentation product for C. scindens is ethanol, while the production of hydrogen, acetate, and formate significantly decreases during growth in minimal media.

=== Hormonal === Vitamin D metabolism is regulated not only by the negative feedback mechanism of calcitriol but also by two hormones: parathyroid hormone (PTH) and fibroblast growth factor-23 (FGF-23). These hormones are essential for maintaining the body's calcium and phosphate balance. Parathyroid hormone (PTH) regulates serum calcium through its effects on bone, kidneys, and the small intestine.Bone remodeling, a constant process throughout life, involves bone mineral content being released by osteoclasts (bone resorption) and deposited by osteoblasts. PTH enhances the release of calcium from the large reservoir contained in the bones. It accomplishes this by binding to osteoblasts, in this way inhibiting the cells responsible for adding mineral content to bones, thus favoring the actions of osteoclasts. In the kidneys, around 250 mmol of calcium ions are filtered into the glomerular filtrate per day, with the great majority reabsorbed and the remainder excreted in the urine. PTH inhibits reabsorption of phosphate (HPO42−) by the kidneys, resulting in a decrease in plasma phosphate concentration. Given that phosphate ions form water-insoluble salts with calcium, a decrease in the phosphate concentration in plasma (for a given total calcium concentration) increases the amount of ionized (free) calcium.
